Check the item history. It's blacked out, so he doesn't have it in his backpack. The one you saw in his backpack is a different one, because he had two of them. Look at the levels and IDs on the featured Chapeau and the Chapeau in his backpack.

The first one you mentioned, maybe he used trade.tf and didn't record that it sold from there. not sure.
