Unique versions of weapons that are unlocked when you start the game either have custom name or description :)
No, unique stock weapons used to come from item drops. A few also came from the oldest crates, such as #1

Where did you hear that from? I was pretty sure that clean, un-named, un-described stock weapons were around for a short time when Valve allowed people to remove the custom name and desc tags after tagging the items. They removed that ability a long time ago, so now it's permanent.
Secondhand knowledge passed from an old friend. He's played since the release so I'm just taking his word for it
